Going Home

Discharge

DischargeWhen your physician decides you are ready to go home, he or she will write a discharge order and inform the nursing team. Your physician or nurse will give you complete instructions about your diet, activity level and medications. If you have any questions, please ask. It is vital that you understand what to do and what not to do once you are home.

Please make arrangements with a family member or friend to help you with transportation home.

Be sure to double-check all areas of your room for personal belongings. When you are ready to leave your room, please tell your nurse, who will arrange a wheelchair escort to the hospital entrance if necessary. Your family member or friend may want to have the car waiting in the short term parking zone that is reserved at the entrance for patient loading.

Social Work Services
